Much Birch Surgery are unable to provide you with a covid-19 vaccination passport or a letter to prove you have been vaccinated against Covid-19.

Download the NHS App to obtain your covid-19 Vaccine passport, or call 119 to request a paper copy to be posted to you (please allow 5 days to reach you).
